ମୁଁ ଏତଦ୍ୱାରା ଘୋଷଣା କରୁଅଛି ଯେ ,  
1.    ସରକାରଙ୍କ ଦ୍ଵାରା ପୁରୀ ଠାରେ ହବିଷ୍ୟାଳିଙ୍କ ନିମନ୍ତେ ବ୍ୟବସ୍ଥା କରାଯାଇଥିବା ଆଶ୍ରୟ ସ୍ଥଳୀରେ ରହିବା ପାଇଁ ଶାରୀରିକ ଓ ମାନସିକ ସ୍ତରରେ ସମ୍ପୂର୍ଣ ସୁସ୍ଥ ଓ ସକ୍ଷମ ଅଟେ ।
2.    ଆଶ୍ରୟ ସ୍ଥଳୀରେ ରହିବା ନିମନ୍ତେ ଧାର୍ଯ୍ୟ କରାଯାଇଥିବା ସମସ୍ତ ନିୟମାବଳିକୁ ମୁଁ ପାଳନ କରିବାକୁ  ପ୍ରତିଶ୍ରୁତି ବଦ୍ଧ ଅଟେ ।  
3.   ମୁଁ ଆଶ୍ରୟ ସ୍ଥଳୀରେ ବିଶୃଙ୍ଖଳିତ ଆଚରଣ ପ୍ରଦର୍ଶନ କଲେ ମୋତେ ଆଶ୍ରୟ ସ୍ଥଳୀ ରୁ ବହିସ୍କୃତ କରାଯାଇପାରିବ ।

4.    ମୋ ଦ୍ଵାରା ଦିଆଯାଇଥିବା ଉପରୋକ୍ତ ସୂଚନା ମୋ ଜ୍ଞାତ ମତେ ସତ୍ୟ ଅଟେ । 
5.   ଉପରୋକ୍ତ ଫର୍ମରେ ଦେଇଥିବା କୌଣସି ବିବରଣୀ ଅସତ୍ୟ ହୋଇଥିଲେ ଏଥିପାଇଁ ମୁଁ ସମ୍ପୂର୍ଣ ଦାୟୀ ରହିବି ଏବଂ ଅଯୋଗ୍ୟ ବୋଲି ବିବେଚିତ ହୋଇ ଆଶ୍ରୟ ସ୍ଥଳରୁ ବହିଷ୍କୃତ ହେବି ।    

I hereby declare that, 
1.    I am physically and mentally fit & fine enough to stay in the accommodation provided by the Government at Puri for the Habishyalies. 
2.    I undertake to abide by all the rules and regulations prescribed for staying in the shelter.
3.    I can be removed from the shelter if I display disruptive behavior at the shelter.

4.    The above information given by me is true to the best of knowledge. 
5.    I will be fully responsible for any false information provided in the above form and will be deemed ineligible and expelled from the shelter.
